- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- What is the difference between and in-network and an out-of-network provider?
- How do I file a claim for an out-of-network provider?

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. If there is less than a 24 hour notice or failure to show up for session than a $85 fee will be applied.
